Humans will always be humans. Mistakes will always be made. The key is to drive measurable behavior change by working with the humans that comprise the workforce. Rather than dropping simulation toolsets into the CISO organization to trip people up and interfere with their productivity, we should be looking at root causes for the errors the humans are making, and helping the people do their jobs well rather than causing them to always second guess themselves.
